Once we receive your information and preferences, an Intake Coordinator will respond within 2 business days to answer questions and discuss options. Depending on their current availability, each of our providers offers a FREE 15 minute phone call to ensure that the counselor you decide on working with is a good fit and the start to a trusting relationship.

When scheduling an intake appointment with one of the providers at Charis, you will be asked for the client’s email and date of birth so that an invite to our TherapyNotes Client Portal may be sent over.

Through the TherapyNotes Portal, you will receive access to and be able to complete all intake forms, and e-sign them on any device. It is through the TherapyNotes Portal that the client will have access to any teletherapy sessions. A PC/MAC is recommended to fill out the intake forms. Click here to access the Client Portal.

If you are unable to access the Portal, or prefer to print your own paper copies of the forms and hand-write your answers, please email your counselor and they can be available for pick up at our Primary Office or mailed to you. Please note that completed portal forms are Charis’ preferred method.

All intake forms are REQUIRED to be filled out and submitted PRIOR to the appointment time to ensure that they have been reviewed by the clinician and to dedicate the intake appointment time to you, rather than to the filling out of paperwork.

Please note that when making use of paper forms, privacy cannot be secured outside of the Charis counseling offices.

Therapy is a collaborative process where you and your Provider will work together to achieve your goals.  This means that you will follow a defined process supported by scientific evidence, where you and your Provider have specific rights and responsibilities.

Participation in therapy is voluntary – you can stop at any time. At some point, you will achieve your goals. At this time, you will review your progress, identify supports that will help you maintain it, and discuss how to return to therapy if you need it.

You and your Provider will discuss what to expect throughout the course of therapy beginning at the intake appointment, including the type of therapy, the length of treatment, and the risks and benefits. If your Provider is practicing under the supervision of another professional, your Provider will tell you about their supervision and the name of the supervising professional.
